- The policy was issued in New Jersey
- The policy was active as of 3/1/20 or issued between 3/2/20-12/31/21
- The policy entered a grace period between 3/1/20-12/31/21
- The policy was canceled or lapsed during that grace period
- The insurance company did not provide the 90-day grace period or repayment options required
